Registration and Insurance Information

Insurance Coverage Questions:

Contact your insurance provider to verify benefits for the type of appointment(s) from the list below for which you have been referred and ask:

  • If your plan is in network with Middle Tennessee Medical Center.
  • If you have any co-pays, co-insurance, deductible or out-of-pocket expenses to meet and what the coverage amount is per calendar year.
  • If pre-authorization is needed.  If so, please notify your Primary Care Physician’s office.

Individual Appointments and Group Classes:

DIABETES GROUP TRAINING:  Includes Basic and Advanced classes as well as Gestational Diabetes classes (CPT Code G0109).

INDIVIDUAL DIABETES TRAINING for pre-diabetes, type 1 and type 2 diabetes, or other diabetes-related complications:  Includes RN consult, Continuous Glucose Monitoring, and insulin/pump instruction.  (CPT Code G0108).

MEDICAL NUTRITION THERAPY:  Includes Advanced Nutrition Class (HCPCS Code 97804) and Individual Diet Consult (HCPCS 97802).

NURSE PRACTIONER PROVIDER VISITS for management (includes facility charge).

If your appointment is for Continuous Glucose Monitoring, ask to verify your disease management benefits for diabetes education including Individual Training (CPT Code G0108) and Continuous Glucose Monitoring (CPT Code 95250).

If your appointment is for pump instruction (CPT Code G0108), please ask which type of pump is preferred by your insurance plan.

Please remember to bring the following to your appointment:

  • Your insurance card(s) and photo ID.
  • The enclosed forms completely filled out to the best of your knowledge.
  • A list of your current medications.
  • Any recent lab work that you may have received from your physician.
  • Blood glucose meter and testing kit.
  • If your appointment is for insulin/pump instruction or management, please bring your insulin vial or prescription as well as your most recent blood glucose record (from the last 7 days).
  • Only one adult support person due to seating limitations in group class.
  • Lunch is provided for 6 hour classes at Baptist and MTMC locations.  If you are scheduled for an alternate, off-site location, please bring a snack or lunch with you to eat in class.
