Medications prescribed by your physician are an important part of staying well — but not if you can’t afford them. Dispensary of Hope - MTMC may be able to help. It is a program that offers help to individuals who are uninsured and have limited income.
How do I qualify?
- You must not have any prescription coverage
- Bring a photo id
- Bring your social security card
- Bring any of the following as proof of income
- Current Tax Return, pay stubs, or bank statement
- SS Disability paperwork
- Social Security award paperwork
- Student loans
- Dated and signed letter from employer
- Unemployment or Pink Slip from employer
- Dependant Verification Form supplied by Dispensary of Hope

- You will receive a medication history card on your first visit. You must bring that at each and every visit.

What medications do you carry?
Please call us at (615) 907-4076 when checking to see what medications we have in stock. We do not carry all medications. We do not carry any controlled substances nor birth control.
